1. Application form in soft copy can be obtained through the Official WhatsApp number 0301-1189725 or collected by hand from FC Teaching Hospital Peshawar, Bara Road Shakas.
2. Application form duly attached with passport size photograph, along with 1x attested photocopy of the credentials/certificates/relevant documents, CNIC, Domicile in a sealed envelope marked with the specific post at the address “COMMANDANT FC TEACHING HOSPITAL, BARA ROAD SHAKAS”, (By courier/ hand).
3. Employees of Govt/Semi-Govt/ Autonomous/ Semi-Autonomous Bodies can apply, however, Departmental Permission Certificates will be required before the interview.
4. No TA/ DA will be admissible over the test/ interview.
5. Eligibility of the candidates shall be determined by Selection Board.
6. Candidate has to provide an equivalence certificate issued by PM&DC/HEC to authenticate the claim of equivalence.
7. Applicants married to Foreigners have to provide a declaration certificate.
8. Experience will only be considered after attaining the prescribed qualifications for the post, the experience certificates should be on prescribed forms of concerned institutes/governing bodies and can be verified.
