1. Candidates must be domiciled in any district of Punjab.
2. Attested copies of educational qualification certificates (Matric, F.Sc and MBBS along with each year’s transcript, Valid PMC/PMDC Certificates) experience certificates, two latest passport size photographs, domicile certificate and CNIC must be attached with the application.
