1. Application form and marks calculation sheet can be downloaded from the website www.skzmdc.edu.pk processing & application fee shall be submitted through Pay Order and Bank Draft only in favour of Deputy Dean Shaikh Zayed Postgraduate Medical Institute, Lahore Account Ma 3154820902 National Bank of Pakistan (NBP) Shaikh Zayed Hospital, Lahore Branch and forms can be sent through couriers on the above-cited form address.
2. Application Forms are also available from the account section of FPGMI, Lahore on Payment of Rs. 2000/-.
3. If the information submitted is found wrong, mismatched or forged, the application will be rejected and a contract will be discontinued no right of appeal at the institution/government level.
4. Applications received after the due date or with incomplete information (marks calculation sheet) shall not be entertained.
5. The last date for submission of the application form on Monday, 23rd August 2021 at 1100AM.
